To play this game you must have a bottle of some beverage (doesn't have to be beer) for the most official you could use sapporo (a beer and also the capitol city of the top island) or medori (a very green beverage, hence medori means green). when you see any of the things on this list in the show you are watching drink the corosponding number of times.
If you are watching any of these shows(un-edited), drink twice (may be doubled if you need it, and most likely will):
Golden boy, Cutey honey flash, sailor moon, X, or any other somewhat perverted shows.
Drink once if there is underware showing, twice if someone is changing, three times if naked, four times if shower scene, five times if sexual acts occur while naked, six times if you find anything worse (at this point you will need it)
Drink once if someone tells a joke and is the only one laughing.
Drink once if a skirt is flipped.
Drink once if a body part jiggles, twice if its intentional.
Drink once if there is blood, twice if disfigured body parts.
Drink once if someone says shit, twice is someone says fuck, three times if they swear three times in one sentence.
Drink once if there is a big costume change scene, twice if sparkly, three times if there is a rhyme to go along with it.

Drink once if at least one character is gay/bi.
Drink once if a person wearing a dress over shoots where she was going and skids turning up her dress.
Drink your friends drink if the show makes a comment as if talking to audience.
Drink once if a character embarasses him/herself by showing off.
Drink once if a character gets drunk, twice if its a girl.
Drink once if a character tries to seduce another, twice if they succeed.
Submitted parts of game
Drink once if you see something blow up, twice if it's a
robot piloted by someone younger than twenty, and four times
if that person is also referred to as anything aside from
intelligent. (Clutzy, cute, a sweet person...)
Take a heavy swig if all the female characters suddenly
start conspiring about something. Two if they're conspiring
about boys who are not their business/actually bad guys.
Drink the whole bottle if you actually see someone wake up
SCREAMING instead of just going 'Ungh!' (Note: Don't even
think that Darien's little dream fit counts. We're talking X
material here.)
Drink once if someone throws a magical attack, misses, and
hasn't the faintest what to do next. (Like try the damn
attack again!)
Drink once if a bad guy is attacking a good guy s/he knows
s/he's losing to, clouds of dust appear, and when they
clear, the bad guy is shocked to realize that the good guy's
still standing. Twice if it's a good guy attacking a bad
guy. Three times if a good guy is intelligent and just
shoots the bad guy with a powerful gun. (You won't see this
unless you watch Outlaw Star or Gundam Wing, I promise you.)
Drink once if somebody says a pathetically translated run on
sentence. (For example, a la Speed Racer: You mean she's the
one whom I have missed all these years that would call my
sister and she is not dead? Ah!)
